Warning signs missed at Muckamore as abuse became ‘normalised’ – inquiry chair

There was "wide-ranging" abuse of vulnerable people at Muckamore Abbey Hospital , including "overuse" of restraint and sedation, the chair of the public inquiry into the case has said.
